Yes, some of the weight you lose on a GLP-1 medication is lean mass, and part of that is muscle. In the SURMOUNT-1 body-composition substudy, about 26% of the weight lost on tirzepatide (Zepbound, Mounjaro) was lean mass, the same share as on placebo. This is a body-composition finding, not a labeled side effect: no GLP-1 label lists muscle loss, muscle pain or muscle cramps among its common adverse reactions.

The trial numbers in detail:

  • Tirzepatide (SURMOUNT-1, 160 people scanned at 72 weeks). Body weight fell 21.3%, fat mass 33.9% and lean mass 10.9% (placebo: 5.3%, 8.2% and 2.6%). Of the weight lost, 74% was fat and 26% was lean mass. On placebo it was 75% and 25%.
  • Semaglutide (STEP 1, 140 people scanned at 68 weeks). Total fat mass fell 19.3% and lean body mass 9.7%. Because fat fell faster, lean mass made up a larger share of the body afterward (up 3.0 percentage points). A 2025 joint advisory from four US nutrition and obesity societies works this out as 5.3 kg of an average 13.6 kg lost (38%) being lean mass.

Lean mass is not the same as muscle. It also includes water, organs and other non-fat tissue. The joint advisory estimates that muscle is about half of lean body mass, so roughly 20% of the weight lost in STEP 1 was muscle.

Why does GLP-1 weight loss take some muscle?

Losing a large amount of weight by any method takes some lean mass with it. The SURMOUNT-1 authors note that diets, other medications and bariatric surgery all show a similar split. The joint advisory names the factors that change how much lean mass goes:

  • How hard and how fast you cut calories. Bigger deficits and faster loss are linked to more lean mass loss.
  • Protein. The advisory says low protein intake because of reduced appetite "may also contribute to muscle loss", especially with older age, menopause, low testosterone, sitting most of the day, or no strength training.
  • Strength training, or the lack of it. Neither SURMOUNT-1 nor STEP 1 used a structured strength program. Participants were told to aim for 150 minutes of activity a week.

Does Zepbound cause muscle pain, aches or leg cramps?

Muscle pain, body aches and leg cramps are not in the common adverse reactions tables of the Zepbound, Mounjaro, Wegovy, Ozempic, Rybelsus or Saxenda (adult) labels. So the labels do not establish them as effects of the drug. Many people still describe aches or night-time leg cramps on tirzepatide and semaglutide, often in the early weeks or after a dose increase.

Some possible contributors are worth ruling out:

  • Fluid loss. Nausea, vomiting and diarrhea are labeled side effects. The labels warn that the dehydration they cause has led to kidney injury. MedlinePlus lists dehydration and low electrolytes (magnesium, potassium, calcium) as causes of muscle cramps.
  • Eating much less. A smaller appetite can mean less fluid and fewer electrolyte-rich foods across the day.
  • New exercise. MedlinePlus lists straining or overusing a muscle as the most common cause of cramps.
  • Tiredness. Fatigue is a labeled side effect of Zepbound and Wegovy. See fatigue on GLP-1s, and back pain if the ache sits there.

How to prevent muscle loss on GLP-1 medication

  1. Set a protein target and hit it most days. The joint advisory reports recommendations of 1.2 to 2.0 g per kg of adjusted body weight per day during major weight loss, or 80–120 g a day as a simpler target. It also says to avoid long periods above 2 g/kg/day. The protein calculator starts GLP-1 users at 1.2 g/kg or more and shows its method in full.
  2. Eat the protein first. The advisory suggests eating protein-rich foods first in a meal, so they get eaten before you feel full.
  3. Spread it out. A position stand from the International Society of Sports Nutrition suggests 20–40 g per serving every 3–4 hours. That was written for people who exercise, not specifically for GLP-1 users.
  4. Use drinkable protein on low-appetite days. Greek yogurt, a smoothie or a shake can be easier than a full plate. See protein shakes for GLP-1 users.
  5. Strength train. The advisory says protein alone is "likely inadequate" to preserve muscle without structured resistance training. It recommends strength training at least 3 times a week plus at least 150 minutes of moderate aerobic activity.
  6. Drink regularly, especially on low-appetite or stomach-upset days. If you have been vomiting or had diarrhea, add fluids with some salt, such as broth. Ask your prescriber before you start magnesium, potassium or other supplements.
  7. Bring up a faster-than-expected drop. The rate of loss and the dose are prescriber decisions. Never skip or change a dose on your own.

If you have kidney disease, ask your clinician before you raise your protein intake. The same protein-and-strength approach also comes up in preventing "Ozempic face".

How to regain muscle mass after GLP-1

The same two levers apply: enough protein and progressive strength training. We found no trial that tested muscle regain after stopping a GLP-1, so there is no GLP-1-specific timeline to quote. The PROT-AGE group recommends at least 1.0–1.2 g/kg a day for people over 65, and 1.2 g/kg or more for those who exercise.

When to call your doctor

Contact your prescriber, or get urgent care if it is severe, for:

  • Cramps that are severe or frequent, last a long time, or don't get better with stretching and fluids (MedlinePlus).
  • Muscle pain with swelling, redness, warmth or weakness.
  • Signs of dehydration with vomiting or diarrhea you can't keep up with. The labels warn this can cause acute kidney injury.
  • Severe belly pain that may spread to your back, with or without vomiting. This is the labeled warning sign of pancreatitis.
  • Low blood sugar if you also take insulin or a sulfonylurea. The labels list this risk.
  • Weakness that makes daily tasks harder (stairs, getting up from a chair), especially if you are older.

What to track and why

Muscle loss and muscle pain are not in GLP AI's side-effect catalog, so add them as a custom symptom with a severity from 1 to 10 and the date. Log your weight so you and your clinician can see how fast it is changing. Log your meals (photo, barcode, typed or voice) to see the protein you actually eat, not the protein you meant to eat. The estimated medication level chart shows where cramps fall in your dose week. The doctor report is a PDF you export and share yourself, with weight, nutrition and symptoms over time. More on logging is in the GLP-1 symptom tracker and the side-effects hub.

GLP-1 Muscle Loss FAQ

Does GLP-1 cause muscle loss?

Some lean mass is lost along with fat, as with any large weight loss. In the SURMOUNT-1 body-composition substudy, 26% of the weight lost on tirzepatide was lean mass, against 25% on placebo. In STEP 1 a 2025 joint advisory calculates about 38% lean mass for semaglutide, roughly 20% of the total being muscle. Muscle loss is not listed as an adverse reaction in any GLP-1 label.

How do I prevent muscle loss on a GLP-1?

Eat enough protein (a 2025 joint advisory reports 1.2 to 2.0 g per kg of adjusted body weight a day during major weight loss, or 80–120 g a day), eat it first in meals, and strength train. The advisory says protein alone is likely not enough without structured resistance training, and suggests strength training at least 3 times a week.

Is muscle pain a side effect of Zepbound?

It is not listed in the common adverse reactions table of the Zepbound label, and muscle pain or spasms are not in the Mounjaro, Wegovy or Ozempic tables either. People do report aches and cramps. Dehydration, low electrolytes and new exercise are possible causes to discuss with your prescriber.

Why do I get leg cramps on tirzepatide?

The label does not list leg cramps, so the cause is not established. Dehydration and low electrolytes are listed causes of cramps in general, and GI side effects plus eating less can lower both. Cramps that are severe, frequent, or come with swelling or weakness need a call to your clinician.

Can you build muscle while on a GLP-1?

The trials did not test a strength program, so there is no GLP-1-specific answer. The joint advisory recommends structured resistance training at least 3 times a week, with adequate protein, to preserve muscle during GLP-1 treatment.
