Yes, GLP-1 medications can make you tired. Fatigue is a listed side effect on the Zepbound, Wegovy and Saxenda labels, reported by 5–11% of people on the drug versus 3–5% on placebo, and on the Saxenda label it was reported mainly in the first 12 weeks, often alongside nausea, vomiting or diarrhea.
So most people on a GLP-1 don't report fatigue, and part of the tiredness people do feel is not the drug directly. Eating far less, low protein, fluid loss and poor sleep are the usual drivers worth checking first. Some tiredness needs a call to your doctor, and that list is below. For other symptoms, see the GLP-1 side effects hub.
How common is GLP-1 fatigue? The label numbers
From the current US prescribing information for each drug. "Fatigue" in these tables is a grouped term: Zepbound counts asthenia (weakness), fatigue, lethargy and malaise; Wegovy counts fatigue and asthenia.
| Drug | What the label measured | Placebo | On the drug |
|---|---|---|---|
| Zepbound (tirzepatide) | Weight-reduction Studies 1 and 2, Table 1 | 3% | 5% (5 mg) · 6% (10 mg) · 7% (15 mg) |
| Mounjaro (tirzepatide) | Type 2 diabetes placebo pool, Table 1 | Not listed in the label's common adverse reactions table | Not listed in the label's common adverse reactions table |
| Wegovy (semaglutide injection) | 2.4 mg weight-reduction trials, Table 3 | 5% | 11% |
| Wegovy 7.2 mg | Table 4 (vs 2.4 mg and placebo) | 5% | 9% (2.4 mg) · 11% (7.2 mg) |
| Ozempic (semaglutide) | Type 2 diabetes trials | Not listed in the label's common adverse reactions table | Named among "other adverse reactions with a frequency of >0.4%" |
| Saxenda (liraglutide) | Adult trials, Table 2 | 4.6% | 7.5% (plus asthenia 2.1% vs 0.8%) |
| Rybelsus (oral semaglutide) | Type 2 diabetes trials | Not listed in the label's common adverse reactions table | Not listed in the label's common adverse reactions table |
One more Mounjaro number: in a cardiovascular outcomes trial of 6,647 adults with type 2 diabetes, fatigue was reported in 10% of people on Mounjaro. That trial compared Mounjaro with another GLP-1 (dulaglutide), not placebo, so it does not show how much of that 10% is the drug.
On Zepbound, fatigue rises with dose (5% → 7%). For the full side-effect picture per drug, see Zepbound side effects and Wegovy side effects.
Why does GLP-1 make you tired?
The labels list fatigue but do not explain it. The likely drivers are indirect, and most of them can be checked:
- You are eating much less. In a 20-week trial, people on semaglutide 2.4 mg ate 35% less at a buffet lunch than people on placebo. Fewer calories is how the drug works. But a big, sudden drop can leave you running on empty, especially on days you barely eat.
- Protein and nutrients drop with the calories. A 2025 joint advisory from four obesity and nutrition societies warns that nutrient shortfalls become more likely at intakes under about 1,200 kcal a day for women and 1,800 for men. It names "fatigue beyond expected levels" as one sign of a true deficiency.
- Fluid loss. Nausea, vomiting and diarrhea cost fluid. Many people also drink less when they eat less. Low fluid can leave you tired and lightheaded.
- GI side effects. On the Saxenda label, fatigue was often reported together with nausea, vomiting or diarrhea. Feeling sick is tiring, and it also makes eating harder.
- Sleep. Poor sleep, or sleep apnea, can be mistaken for a drug side effect. Zepbound is FDA-approved to treat moderate to severe obstructive sleep apnea in adults with obesity. If you snore loudly or wake unrefreshed, that is worth raising.
How long does GLP-1 fatigue last?
The Saxenda label says fatigue and weakness events were reported mainly within the first 12 weeks of treatment. The Zepbound label says most nausea, vomiting and diarrhea happened during dose escalation and decreased over time. Fatigue often travels with those symptoms, so a tired week after a dose increase is a common pattern.
Some people notice tiredness in the first day or two after their weekly shot. The labels do not report that timing, so treat it as something to check in your own log, not a rule. Fatigue that keeps getting worse, or does not ease after several weeks on a steady dose, is a reason to talk to your prescriber.
How to combat GLP-1 fatigue
- Eat something every few hours, even when you are not hungry. The advisory suggests a small breakfast and then small meals every 3–4 hours. A few bites of something with protein beats skipping a meal.
- Put protein first. Higher targets of 1.2–1.6 g per kg per day have been proposed during active weight loss, and intake should not fall below 0.4–0.5 g/kg. Your number depends on your body, so use the protein calculator and confirm the target with your clinician, especially if you have kidney disease. Low protein also feeds muscle loss.
- Drink fluids through the day. Sip steadily rather than all at once, which is easier on a full stomach. Add electrolytes on days with vomiting or diarrhea.
- Treat the nausea, not just the tiredness. Smaller, low-fat meals help both. See GLP-1 nausea for the full list.
- Protect your sleep. Keep a regular bedtime, and note if sleep got worse after you started. That is a separate issue covered in GLP-1 insomnia.
- Ask your prescriber before adding supplements or other medicines. That includes iron or vitamin pills and caffeine pills. If fatigue hits hard at each dose step, ask whether your titration plan fits you. Any change to your dose is their decision.
When to call your doctor
Tiredness is common, but some forms of it are a warning sign:
- You take insulin or a sulfonylurea (such as glipizide or glimepiride). GLP-1s lower blood sugar. In a Zepbound trial of people with type 2 diabetes, low blood sugar affected 10.3% of those also on a sulfonylurea, versus 2.1% of those who were not. The Wegovy Medication Guide lists signs such as weakness, confusion or drowsiness, shakiness, sweating, dizziness, hunger and a fast heartbeat. Check your glucose if you can and follow your low-blood-sugar plan.
- Signs of dehydration: dizziness, very little or dark urine, or vomiting or diarrhea that does not stop. The labels warn that fluid loss from GI side effects can lead to kidney problems, and some cases needed dialysis.
- Severe or ongoing stomach pain, sometimes spreading to the back, with or without vomiting. This is the labeled warning sign of pancreatitis. Pain in the upper right belly, fever, or yellow skin or eyes can point to gallbladder problems.
- Fainting, chest pain or shortness of breath. Seek urgent care.
- Fatigue that keeps getting worse, or comes with low mood or loss of interest. See mood, anxiety and depression, and if you have thoughts of harming yourself, call or text 988 in the US.
Fatigue can also come from causes unrelated to the drug, such as anemia or thyroid problems. Ask your doctor whether tests make sense.
What to track and why
"I'm always tired" is hard to act on. "Tired at 6/10 on days 1–2 after each shot, worse on days I ate under 60 g of protein" gives your prescriber something to work with.
In GLP AI, log fatigue from the side-effect catalog with a severity from 1 to 10, next to your shots, meals and water. The estimated medication level chart shows where each entry falls in your dose week. Before an appointment, export the doctor report PDF and bring it, along with the questions in our doctor visit prep guide. For a full overview of logging symptoms, see the GLP-1 symptom tracker.
GLP-1 Fatigue FAQ
Does Zepbound cause fatigue?
Yes, it is a labeled side effect. In the Zepbound weight-reduction trials, fatigue (including weakness, lethargy and malaise) was reported by 5% of people at 5 mg, 6% at 10 mg and 7% at 15 mg, versus 3% on placebo.
Does Wegovy cause fatigue?
Yes. The Wegovy label reports fatigue or weakness in 11% of people on the 2.4 mg injection versus 5% on placebo, and 11% on the 7.2 mg dose.
How long does GLP-1 fatigue last?
The Saxenda label reports it mainly in the first 12 weeks, and GI side effects on the Zepbound label mostly occurred during dose escalation and eased over time. Fatigue that worsens, or does not ease after several weeks on a steady dose, is worth raising with your prescriber.
Why am I so tired on tirzepatide?
Common drivers are eating far fewer calories, low protein, too little fluid, nausea or diarrhea, and poor sleep. If you also take insulin or a sulfonylurea, tiredness, shakiness or confusion can be low blood sugar, so check your glucose and contact your care team.
How do you combat GLP-1 fatigue?
Eat small meals every 3 to 4 hours, put protein first, drink fluids through the day, manage nausea, and protect your sleep. Ask your prescriber before adding supplements or changing anything about your dose.
